Objectives: Mango supplemented with probiotics was previously reported to synergistically attenuate inflammation and improve cognitive function in obese individuals. Therefore, the objectives are to further investigate the effects of mango and probiotics on gastrointestinal hormones, apolipoproteins, and gut health in lean and obese individuals. It was hypothesized that mango and probiotics would synergistically increase metabolite bioavailability and beneficially modulate apolipoproteins, gut hormones, and microbiota.

Methods: This double-blind, randomized, placebo-controlled trial was conducted in healthy lean (n=50; BMI 18-23 kg/m2) and obese (n=44; BMI 27-35 kg/m2) adults between 18-65 years. Participants received 400 g cubed, fresh mango with one placebo/probiotics capsule daily for 8 weeks. Anthropometrics, fasting blood, urine, and stool samples were collected at weeks 0 and 8. To further investigate metabolite bioavailability on intestinal hormones and microbiota, a sub-analysis was conducted on the best and worst mango metabolite producers from the obese-probiotics group.

Results: After 8 weeks of mango and probiotics, polyphenol metabolite production increased in the lean and probiotics groups. However, mango with probiotics had no effect on apolipoprotein concentrations. Lower apolipoprotein A1 (p=0.0149) and higher leptin concentrations (p < 0.0001) were observed in the obese group at week 0, but there were no differences at week 8. Further analyses revealed obese individuals with the fewest mango metabolites exhibited low levels of peptide YY (p=0.0317) with higher total ghrelin (p=0.0317) and IL-1β levels (p=0.0159). Additionally, the relative abundance of Firmicutes phylum and Clostridiaceae family increased in the low metabolite producers.

Conclusions: Mango supplemented with probiotics increased gallotannin-metabolites in the lean and probiotics group but did not affect apolipoproteins or gut hormones excluding leptin, which was reduced in the obese group by week 8. Gut hormones and microbiota were not beneficially impacted in the low metabolite producers likely from decreased bioavailability of polyphenols. These findings indicate mango and probiotics may provide a protective effect by attenuating leptin and maintaining appetite regulation and intestinal microbiota.
