Objectives: We aimed to assess the independent and combined associations of oral microbiome diversity and dietary quality with risks of all-cause and cause-specific mortality.

Methods: We used a prospective cohort study of US National Health and Nutrition Examination Survey (NHANES, 2009-2012), including 7988 eligible adults aged over 30 years. Oral microbiome α- diversity was measured with metrics including the Simpson Index, observed amplicon sequence variants (ASVs), Faith’s Phylogenetic Diversity (PD), the Shannon-Weiner index. Dietary quality was assessed using Healthy Eating Index-2015 (HEI-2015). All-cause mortality, cardiometabolic mortality, cancer mortality, other disease mortality identified using linkages to death registry from until December 31, 2019. Cox proportional hazard models are used to assess the relations of oral microbiome α- diversity with the outcomes.

Results: The mean (SD) age of the study population was 43.1 (13.8) years, and 50.6% of them were female. During a mean follow-up of 8.8 years, a total of 437 death cases were documented. When adjusted for socioeconomic status and lifestyle factors, we observed independent associations of α- diversity and diet quality with all-cause mortality. Comparing participants at top tertiles to those at bottom ones, the corresponding HRs (95%CIs) were 0.67 (0.52, 0.85) for Simpson Index, and 0.70 (0.54, 0.90) for HEI-2015, respectively. The lowest hazard of mortality (HR=0.40, 95%CI, 0.25, 0.63) was observed among participants with the highest level of α- diversity (in Simpson Index) and HEI-2015. Moreover, all α- diversity indices were associated with lower risk of mortality from cardiometabolic disease, cancer, and other diseases.

Conclusions: Higher α- diversity of oral microbial and dietary quality were independently associated with lower risks of mortality. Future studies are needed to confirm our findings and explore the biological pathways.
