Objectives: The Nutrition for Precision Health (NPH) Study is the first ancillary study of the All of Us Research Program. The NPH Consortium consists of investigators from 15 funded centers across the US and the National Institutes of Health (NIH). The NPH Study is discovery science that will investigate individual responses to diets and foods. One goal of NPH is to evaluate novel dietary assessment methods for accuracy and usability to improve dietary assessment methodology and inform use of different dietary assessment tools in various subpopulations.

Methods: NPH consists of three modules conducted across six US clinical centers. Module 1 is a cross-sectional study that will recruit ~10,000 adults aged ≥18 y enrolled in All of Us. Various parameters including dietary intake in the participants’ natural setting will be measured over 10 d. Module 2, a dietary intervention in ≥1500 adults who participated in Module 1, will use a randomized crossover design to provide three different diets in a community-dwelling setting for 14 d each, separated by ≥14 d each. Module 3 will also use a randomized crossover design to test the same three diets over 14 d each in a domiciled setting in ≥500 adults who participated in Module 1. Different people will participate in Modules 2 and 3. Dietary assessment tools used in Modules 1 and 2 will be the Diet History Questionnaire (DHQ) III, the Automated Self-Assessment Dietary Assessment Tool (ASA24-2022) for 24h dietary recalls, and two image based methods: the mobile Food Record (mFR) and the Automatic Ingestion Monitor (AIM)-2. Module 3 will include doubly labeled water to provide a reference value for comparing energy intake from the tools used in Modules 1 and 2, and two additional tools – the ASA24 dietary record and image-assisted 24h recall (mFR+ASA24 recall). In each module, tool usability and preference will be assessed. Analysis will be conducted to assess usability, preference and accuracy across subpopulations and relate dietary intake to other NPH outcomes.

Conclusions: NPH is the largest, most comprehensive study to evaluate and compare multiple dietary assessment tools including novel image-based tools, for accuracy, preference and usability across sex, age and socioeconomic groups in natural and domiciled settings and will yield information for future use of the different tools.
