Objectives: After developing a culturally tailored GDM-DH app designed to support management and treatment of gestational diabetes (GDM) among Nepalese women, we examined its usability, feasibility and acceptability in an exploratory pilot randomized clinical trial (RCT).

Methods: We recruited 60 women newly diagnosed with GDM from Dhulikhel Hospital in Dhulikhel, Nepal, and randomized them to either GDM-DH app + standard care, or standard care alone, from 27±3 gestational weeks (GW) to delivery. Primary outcomes in this pilot RCT were app engagement, and usability and acceptability of the GDM-DH app. At 36.3+2.7 GW, the well-validated 10-item System Usability scale (SUS) and the 21-item Mobile App Usability Questionnaire (MAUQ) were administered to the intervention group to assess the usability of the GDM-DH app; the 9-item Oxford Maternity Diabetes Treatment Satisfaction Questionnaire (OMDTSQ) was administered to both groups to assess satisfaction with GDM care.

Results: Recruitment and retention rates for the RCT were 84.5% and 93.3% respectively. In the intervention group (n=30), we demonstrated good engagement with the app, supporting the feasibility and acceptability of the GDM-DH intervention. The average time spent on the app was 19 minutes/day. The mean score on SUS was 72.2 ± 5.9 (possible scores 0-100), indicating good usability (scores > 68 indicate good usability) of the GDM-DH app. On the SUS, 95.6% of the intervention group reported that they would like to use the app frequently, 95.8% said the app was easy to use, and 91.6% found the various functions in this app to be well integrated. On the MAUQ, the mean score was 50.4 ± 9.7 (possible scores 21-147; lower scores are better); 96.2% agreed that the app was easy to learn to use, 100% were satisfied with the app and 96.4% said they would use the app again. On the OMDTSQ, patient satisfaction scores (possible scores 0-27) were significantly greater for the women who used GDM-DH app (mean scores 16.1±3.7 and 13.5±6.0 for the intervention and control groups respectively, p = 0.047).

Conclusions: These findings support the feasibility, usability and acceptability of the GDM-DH app among our target users of Nepalese women with GDM. A larger RCT is warranted to verify these findings and further investigate the clinical utility of this app.
