(OR14-02-23) Time Restricted Eating Without Calorie Counting for Weight Loss in a Racially Diverse Population: A 12-Month Randomized Controlled Trial

Objectives: Time restricted eating (TRE), without calorie counting, has become a popular weight loss strategy, yet no long-term randomized trials have evaluated its efficacy. The objective of the study is to determine whether TRE is more effective for weight control and cardiometabolic risk reduction, compared to calorie restriction (CR) or controls.

Methods: Adults with obesity (n = 90; age 40 ± 11 y; 33% Non-Hispanic Black, 46% Hispanic) were randomized to 1 of 3 groups for one year: 8-hour TRE (eating between 12-8 pm, without calorie counting), CR (25% energy restriction daily), or non-treatment control (eating over >10 h per day). The primary outcome was change in body weight by month 12.

Results: Seventy-seven participants (86%) completed the 12-month study. TRE participants were compliant with their eating window on 87% of days over 12-months.Results from the intention-to-treat analysis show that weight loss was -4.61 kg [95% CI, -7.37, -1.85; P = < 0.01] for the TRE group and -5.42 kg [95% CI, -9.13, -1.71; P = < 0.01] for the CR group, relative to controls, with no significant difference between the TRE and CR groups. Change in fat mass, lean mass, visceral fat mass, waist circumference, bone mineral density, blood pressure, plasma lipids, insulin sensitivity (QUICKI) or HbA1c were not associated with treatment group in any pairwise comparisons at month 12. However, TRE was associated with increases in insulin sensitivity (0.02 [95% CI, 0.01, 0.04]) compared with control, but not compared with CR.

Conclusions: TRE is an effective nutrition strategy for weight loss and insulin sensitivity improvements, when compared to controls eating over a period of 10 or more hours, but not versus daily CR, in a racially diverse population.
