Objectives: To investigate the effect of daily cinnamon spice supplementation, rich in polyphenols, on glycemic response using non-invasive continuous glucose monitoring in subjects with obesity and prediabetes.

Methods: This is a randomized, controlled, double-blind, crossover trial among 18 obese participants (age 51.1years; fasting plasma glucose 102.9mg/dL), who started with a 2-week run-in phase consuming a low polyphenol ‘beige diet”. The participants were randomized to take cinnamon (16 capsules-4 grams of cinnamon per day, the cinnamon species (C. Burmanii) or Indonesian cinnamon were used and were placed into 250 mg capsules), or placebo, followed by 2 weeks of washout with a beige diet, and crossover to cinnamon or placebo for 4 weeks. Cinnamon spice (Indonesia) was purchased at the local grocery store (McCormick®) and contained 3.9% proanthocyanin. A continuous glucose monitoring system (FreeStyle CGM, Abbott Diabetes Care, Alameda, CA), was used throughout the entire study for each participant to assess glucose dynamics (694 follow-up days in total; 66,624 glucose observations). Participants completed daily digestive symptoms logs. We performed a mixed-model analysis over time as a within-subject test and the intervention as a between-subject factor, and the primary outcome was the glucose levels.

Results: The overall, repeated glucose levels for 24hr were significantly lower in the cinnamon intervention compared with the placebo after 4 weeks of intervention for each group (mixed models; P< 0.001). The mean netAUC was significantly lower in the cinnamon vs. the placebo group (24hr; p=0.01). In addition, the cinnamon supplement led to a lower glucose peak compared to placebo (Δpeak 9.56±9.1 vs. 11.73±8.0 mg/dL; p=0.027). There were no differences in the digestive symptoms (abdominal pain, borborygmi, bloating, excess flatus, number of stools/day) between the cinnamon and the placebo supplementations, and as compared to baseline.

Conclusions: Cinnamon supplementation to our daily nutrition has a beneficial glycemic effect in prediabetes.
