(P08-016-23) Comparison of Self-Reported and Actigraphy-Derived Sleep Measures for Patients Receiving Home Parenteral Nutrition

Objectives: Patients receiving home parenteral nutrition (HPN) frequently report disrupted sleep, however there are often inconsistencies between self-reported and objectively measured sleep. The aim of this study was to compare sleep measures estimated from self-report and wrist actigraphy in adult patients receiving HPN.

Methods: In this secondary analysis, we pooled data from two sleep-related studies enrolling adult patients receiving HPN. We compared sleep responses to the Pittsburgh Sleep Quality Index (PSQI) questionnaire against comparable measures from 7-day averages of wrist actigraphy using the Philips Actiwatch Spectrum PRO. Sleep measures included bedtime, wake time, time in bed, total sleep time (i.e., sleep duration), and sleep onset latency (i.e., the amount of time it takes to fall asleep). We calculated Spearman’s correlation coefficients for each set of sleep measures to assess the similarity between the two assessment methods.

Results: We analyzed a total of 35 participants. Participants had a mean age of 52 years, a mean BMI of 21.6 kg/m², and 77% identified as female. Participants reported an average bedtime of 23:02, wake time of 07:59, total sleep time of 6.5 hours, time in bed of 9 hours, and sleep onset latency of 23 minutes. We found the highest correlations for wake time (r =0.90; p < 0.001) and bedtime (r =0.74; p < 0.001). We observed lower correlations for time in bed (r =0.58; p < 0.001) and sleep onset latency (r =0.40; p =0.019). We found the lowest correlation for total sleep time (r=0.35; p =0.042). In general, compared to actigraphy, self-reported measures were earlier for bedtime and wake time, longer for time in bed and sleep onset latency, and shorter for total sleep time.

Conclusions: In this sample of adults receiving HPN, we observed high to moderate correlations for sleep measures from self-report with wrist actigraphy. Weaker correlations for total sleep time and sleep onset latency may indicate poor sleep recall or low actigraphy sensitivity in this population. Follow-up research is needed to assess the role of demographic characteristics and perceived sleep quality on the estimated correlations.
