Objectives: Ultra-processed food consumption has been associated with cardiometabolic diseases which are associated with poor prognosis in women with breast cancer (BC). We investigated UPF intake in relation to cardiometabolic risk factors in BC survivors following a Mediterranean diet.

Methods: The study population consisted of 506 women (mean age 52±9 years) with a BC diagnosis (stages I-III) enrolled in the multicentre lifestyle modification trial DEDiCa conducted in Italy. At baseline we collected data on weight, height, waist circumference (WC), systolic blood pressure, fasting levels of glycemia, serum low- and high-density lipoprotein cholesterol (LDL-C, HDL-C, respectively), triglycerides and hemoglobin A1c. UPFs were derived from participants’ 7-day diet records and classified according to group 4 of NOVA classification. Multiple logistic regression analyses with odds ratios (OR) and corresponding 95% confidence interval (CI) were used to explore the odds of being within a low risk cardiovascular target according to UPF median intakes.

Results: UPF consumption correlated directly with intakes of total energy (r=0.46, p < 0.0001) and saturated fatty acids, SFA (r=0.58, p < 0.0001), while inversely with dietary fiber/1000 kcal (r=0.36, p < 0.0001). Consumption of UPFs above the median (58g/day) resulted in significantly lower odds of being in the low LDL-C target < 116 mg/dL (OR 0.65, 95% CI 0.43-0.97) and a tendency to have lower odds for reaching target values of HDL-C, HbA1c and WC.

Conclusions: Higher consumption of UPFs were associated with unfavorable cardiometabolic profiles and specifically a lower likelihood of being within LDL-C target values. These finding are relevant in view of the greater cardiovascular risk in BC survivors.
