(P22-053-23) An “N-of-1” Study Protocol for Investigating Inter-Individual Variations in Glycemic Responses to Popular Diets in Healthy Male Participants

Objectives: This protocol outlines a novel method for investigating inter-individual variations in glycemic responses to four popular diets in healthy male participants. The aim of the study is to compare the glycemic responses of participants to a high carbohydrate-high glycemic index diet (CON) against diets with high-carbohydrate low glycemic index (LGI), high-protein (HP), and high-fat (HF) using multiple single-participant trials. The secondary objective is to identify the factors contributing to inter-individual variability by conducting deep metabolic phenotyping and multi-omics profiling of each participant and fine mapping of their glycemic responses to different diets. The hypothesis is that glycemic responses to the diets will vary and be influenced by personalized characteristics as well as the modulation of metabolites secreted by gut microbiota, their genomic composition.

Methods: The proposed study is a randomized, crossover trial that aims to investigate the effects of three different diets (LGI, HP, and HF) on the glycemic responses of 120 participants compared to CON diet. Over a course of 2 weeks, participants will be provided with meals that have a fixed macronutrient composition. The study design is a "n-of-1" approach, where each participant serves as their own control, rather than a blanket approach. The primary outcome will be the measurement of interstitial glucose using a continuous glucose monitoring system. In addition, the study will analyse the gut microbiota obtained from faecal samples and perform metabolomics analysis on blood and gut microbiota samples before and after the dietary intervention.

Conclusions: Analysis from multiple single participant trials for blood glucose management will provide valuable insights into the factors that influence inter-individual variability in response to different diets. This protocol will inform the design of future clinical trials and the development of personalized nutrition interventions aimed at optimizing blood glucose control.
