Objectives: To investigate the prevalence of vitamin K dependent coagulopathy in hospitalized patients during the Omicron wave of COVID-19 pandemic, and the role of vitamin K in COVID-19 related coagulation disorder.

Methods: This multiple-center study was performed at the Third Chengdu Hospital, Sichuan, China, and Central Hospital of Nanchong, Sichuan, China. We performed a retrospective evaluating of coagulation screening test during a 4-week period (10/12/2022-10/01/2023) around the Omicron wave in Sichuan, China. We selected the cases with prolonged activated partial thromboplastin time (APTT), elevated International Normalized Ratio (INR) and normal thrombin time (TT), Fibrinogen(Fib), which indicated vitamin K dependent coagulopathy. Data was collated from the electronic patient records, which included patients’ characteristics, coagulation indexes, PCR or rapid antigen test for COVID-19, drug compliance, co-medications, liver function and antibiotics consumption. We compared these to the results from the same period during the COVID-19 Lockdown (10/12/2021-10/01/2022).

Results: 1. During the Omicron wave period (10/12/2022-10/01/2023), 229/12145(0.188‰) hospitalized patients were identified as vitamin K dependent coagulopathy, compared to 158/11036（0.116‰）patients shown vitamin K dependent coagulopathy in previous year (10/12/2021-10/01/2022)( OR 1.32, 95% CI 1.12-1.54; p<0.05).
2. Fifty-six covid-19 infected patients who were taking long term vitamin K antagonists (AVK) with stable International Normalized Ratio (INR) before infection showed vitamin K dependent coagulopathy, which means overdose of AVK during infection period. Vitamin K dependent coagulopathy also occurred in forty-one Covid-19 infected patients without any coadministration of anticoagulants. Among the 41 patients, 18 patients received vitamin K1 treatment with all showed normalization of coagulation of coagulation indexes.

Conclusions: Hospitalized patients during Omicron wave of COVID-19 pandemic showed increased vitamin K dependent coagulopathy. compared with the none COVID-19 pandemic period. Enhanced vitamin K expenditure might contribute to the AVK overdose in INR stable patients with COVID-19 infection.
