(P15-018-23) Association of Eating Jet Lag With Circadian Typology, Adiposity and Blood Glucose Levels Among People With Diabetes Type 2

Objectives: The effects of chrononutrition have been associated with obesity and adverse metabolic outcomes independent of meal size and composition. However, the impact of eating jet lag i.e. variability in the timing of food intake between weekends and weekdays on circadian typology, adiposity and blood glucose levels among people with Diabetes (PWDs) remains unexplored.

Methods: A sample size of 227(age: 35-60 years, yrs of diabetes : < 5, no comorbidities) was calculated considering the prevalence of diabetes in the district, India. Eating window, breakfast skipping, evening latency, midnight eating and largest meal were assessed. Body composition, dietary intake, circadian typology and sleep were assessed using BIA,72 hours food records, Horne and Ostberg’s Morningness-Eveningness questionnaire, PSQI respectively. Glycaemic control was assessed using HbA1c, FBS, and PP2BS. Insulin resistance was assessed by fasting TyG index. Eating jet lag was calculated in hours as Eating midpoint on weekends − Eating midpoint on weekdays. The variability in the timing of meals was calculated as breakfast or lunch or dinner jet lag (h) = Time on weekends − Time on weekdays.

Results: A total of 4086 meals from 681 days of dietary records were studied. Eating jet lag was positively correlated with blood glucose levels and TyG index. The threshold eating jet lag by restricted cubic splines was 4 hrs, from which the FBS could significantly increase among PWDs. The timing of breakfast, lunch, and dinner and eating midpoint between weekdays and weekends were significantly different (p  < 0.05). There was significant variability in breakfast and lunch time than dinner and was significantly associated with social jetlag (p < 0.05). Evening chronotypes had higher variability in meal timing during weekends as compared to morning yet the association between HbA1c and eating jet lag was independent of the chronotype.87% of males and 80% of females were obese and had higher eating jet lag than PWDs with normal BMI.

Conclusions: The study indicates that greater the eating jet lag, greater is the adiposity and blood glucose levels among PWDs, independent of chronotype and social jetlag. To our knowledge, this is the first study exploring the relationship between eating jet lag with adiposity and blood glucose levels among PWDs and provides the framework for future experimental studies.
