Objectives: Type 2 diabetes mellitus (T2DM) has been regarded as a chronic metabolic dysfunction disease, and poor course control will lead to diabetes ‘Metabolic Memory’ eventually pointing to various diabetes complications. Clinical studies have found that low folate status is positively correlated with the complication rate of diabetes exacerbation on T2DM patients, but the detailed association and mechanism are still unclear. The purpose of this study was to clarify whether low folate (LF) has the effect of promoting the metabolic process of diabetes, and to explore the regulatory potential of marginal folate deficiency and diabetes mellitus with metformin as a positive control group.

Methods: THLE-2 human liver cells were cultured with high glucose for 4, 8, 12, and 16 days. We monitored the microenvironment factors, including insulin, intracellular glucose and folic acid concentrations to mimic the in vivo microenvironment in the late stage of T2DM.

Results: The results showed that folate malnutrition reduces intracellular glucose utilization, increases insulin resistance, and modulates the glucose-energy metabolism related factors, lactate and NADPH. Also, folate malnutrition negatively regulates the efficacy of Metformin.

Conclusions: Taken together, low folate nutrition may promote insulin resistance by reprogramming glucose energy metabolism.
