Objectives: To examine the association of insomnia with diet quality and unhealthy eating behaviors and to what extent the insomnia-diet relationship was mediated by the presence of these eating behaviors.

Methods: Included were 50,218 Chinese adults (mean age: 49.9 ± 14.6 y) who were free of diabetes, cardiovascular diseases, and cancer in 2014, when information on insomnia symptoms, dietary intake and presence of night-eating behavior and skipping breakfast was collected via questionnaires.

Results: Individuals with insomnia had lower total modified Alternative Healthy Eating Index (mAHEI) score (adjusted mean difference= -1.14; 95% confidence interval (CI): -1.48, -0.80), and higher odds of having behaviors of night-eating (adjusted OR=1.90, 95% CI: 1.69, 2.14) and breakfast-skipping (adjusted OR= 1.41, 95% CI: 1.24, 1.62), relative to those without insomnia, after adjusting for potential confounders, such as age, sex, total energy, and health conditions (P-difference < 0.001 for all). The presence of these unhealthy eating behaviors significantly mediated the association between insomnia disorder and mAHEI score, with a mean proportion of 6.6% for night-eating and 2.6% for breakfast-skipping.

Conclusions: Insomnia was associated with poorer diet quality and higher odds of having unhealthy eating behaviors in Chinese adults.
