Objectives: We examined the association between usual protein intake, estimated by the Ensemble method based on marginal-effect models (EMM) and the National Cancer Institute method (NCI), and the prevalence of renal hyperfiltration (RHF) among healthy Korean adults.

Methods: We included a total of 20,148 adults in the Korean National Health and Nutrition Examination Survey (KNHANES) from 2008-2011 and 2017-2018. Dietary animal and plant protein intakes were assessed using the one-day 24hr recalls. The usual intake was estimated using two-day 24hr recalls from 2009 (for 2008-2011) and 2019 (for 2017-2018). We used two statistical models to adjust for day-to-day variation: EMM and NCI. Energy- and year-adjusted protein intake was grouped into sex- and year-specific quintiles. We calculated the estimated glomerular filtration rate (eGFR) using the Chronic Kidney Disease Epidemiology equation based on age, sex, ethnicity, and serum creatinine level. RHF was determined as eGFR with residuals exceeding the 95 percentile after adjusting for age, sex, height, and weight. Odds ratio (OR)s and 95% confidence interval (CI)s were estimated using the weighted multivariate logistic regression models.

Results: We found a lower prevalence of RHF among participants with low intake of animal protein in the EMM model. When protein intake was replaced for carbohydrate intake, ORs (95% CIs) in the first quintile were 0.77 (0.59-1.00) for plant protein and 0.63 (0.47-0.83) for animal protein compared to the second quintile. ORs for higher quintiles compared to the second quintile were not statistically significant. The associations were less clear in the one-day method or NCI method. ORs (95% CIs) comparing the first with the second quintile were 0.83 (0.65-1.06) for plant protein and 0.91 (0.69-1.21) for animal protein in the NCI method. ORs (95% CIs) for the same comparison were 0.84 (0.66-1.06) for plant protein and 1.14 (0.88-1.48) for animal protein in the one-day method.

Conclusions: Low usual intakes of animal and plant proteins may be associated with a lower prevalence of RHF among healthy Korean adults.
