Objectives: Age-related muscle loss is associated with muscle synthesis decrease and physical activity decrease. This study aimed to examine the effects of Agastache rugosa (AR) and supervised resistance exercise in healthy middle-aged subjects.

Methods: A 12-week, randomized, double-blind, parallel, placebo-controlled trial were conducted with 37 healthy adults. The subjects aged 50-75 were randomly assigned to either AR (1 g/day) or placebo and they were supervised whole body resistance exercise twice a week for 50 minutes per day. We measured body composition including anthropometric parameters, muscle mass and skeletal muscle mass by using Inbody at baseline and the last visit. A linear mixed-effect model was used to test differences of safety and effectiveness between AR and placebo groups.

Results: 37 eligible subjects were randomized to placebo group (n = 18) or AR group (n = 19). After the intervention, right, left and trunk lean mass were significantly increased (p < 0.05) in AR group. The AR also improved body composition index, showing a significant rise in fat free mass index (p=0.033), skeletal muscle index (p=0.040) and nasal metabolic rate (p=0.049). No serious adverse events and changes of safety biomarkers were observed in both groups.

Conclusions: These finding suggest AR supplementation with supervised resistance exercise might improve muscle synthesis and body composition in healthy middle-aged adults.
