Objectives: To investigate whether adherence to a plant-based heart-protective diet might attenuate risks of cardiovascular events in the UK Biobank (UKBB) population.

Methods: This study included 192,274 individuals (mean age: 56.3±7.9 years, 43.3% males) in UKBB who: (1) were free of cardiovascular diseases (CVDs) at baseline; and (2) performed at least one 24-h dietary measurement between 2009 and 2012. Using dietary data collected by the Oxford WebQ, we developed a heart-protective diet score (HPDS) based on 22 food groups. The adherence to an HPDS was divided into quartiles, where Q1 (reference group) and Q4 represented the lowest and the highest adherence, respectively. Associations between adherence to a heart-protective diet and cardiovascular outcomes were examined using Cox proportional hazard models.

Results: After a median (IQR) follow-up of 12.3 (11.7, 13.2) years, compared to Q1, participants who had the highest HPDS (Q4) had an 8% decreased risk of CVD incidence (HR: 0.92 [0.88,0.95]), 12% decreased risk in ischemic heart disease incidence (IHD; HR: 0.88 [0.83,0.93]), 18% decreased risk in myocardial infarction (MI; HR: 0.82 [0.74,0.90]), and 4% decreased risk in heart failure (HF; HR: 0.86 [0.78,0.95]), respectively, adjusting for age (timescale), sex, ethnicity, Townsend deprivation index, socioeconomic status, lifestyle factors (e.g. physical activity, sleep, smoke habits), and history of cancer and hyperglycemia/type 2 diabetes diagnosis. In addition, we observed a more pronounced protective effect in CVD mortality in individuals with the highest HPDS (Q4), with a 28% decreased risk of overall CVD mortality (HR: 0.72 [0.61,0.86]), 32% decreased risk in IHD mortality (HR: 0.68 [0.52,0.89]), 35% decreased risk in MI mortality (HR: 0.65 [0.46,0.91]), and 49% decrease risk in HF mortality (HR: 0.51 [0.30,0.85]).

Conclusions: Adherence to a plant-based heart-protective diet is associated with a significant attenuation of cardiovascular disease incidence and mortality.
