Objectives: We identified foods associated with plasma enterolactone (ENL) concentrations and whether ENL concentrations were associated with plasma concentrations of inflammatory markers in breast cancer survivors in Korea.

Methods: Non-fasting plasma ENL concentrations were quantified from 431 female breast cancer survivors aged 30-78 years, and their daily dietary intakes (n=422) were assessed using the food frequency questionnaire (FFQ) or 3-day dietary record (DR)s. Plasma ENL and inflammatory markers were log or box-cox transformed to improve normality, and adjusted for two measurement time points using residual method. Least-squares mean (LS-mean)s and 95% confidence interval (CI)s were estimated for ENL concentrations according to quantiles of intakes of food groups using the generalized linear models. The composite z-score was calculated by summing the z-scores of each inflammatory marker (adiponectin was included with a negative sign). Logistic regression models were used to estimate the odds ratio (OR)s and 95% CIs for binary outcomes of the composite z-score defined as 75th percentile or higher.

Results: Higher consumption of nut and seed was associated with elevated concentrations of ENL; LS-means (95% CIs) of the lowest and the highest quartiles were 6.91 (4.50, 10.37) and 10.70 (6.98, 16.16) nmol/L (p for trend =0.015), respectively. Fruit intake was inversely associated with ENL concentrations; LS-means (95% CIs) of the lowest and the highest quartiles were 9.29 (6.12, 13.88) and 5.77 (3.61, 8.95) nmol/L (p for trend =0.028), respectively. Participants who consumed one or more cups of coffee had higher ENL concentrations compared to non-drinkers; LS-means (95% CIs) were 6.91 (4.47, 10.44) for non-drinkers and 9.66 (6.56, 14.03) nmol/L for ≥ 1 cups of coffee (p for trend =0.046). ENL concentrations were not significantly associated with the composite z-score or individual inflammatory markers. For binary outcome of the composite z-score, OR (95% CI) for the highest quartile compared to the lowest quartile of ENL concentration was 0.70 (0.34, 1.42; p for trend =0.308).

Conclusions: In this cross-sectional study of breast cancer survivors, ENL concentrations were positively associated with nut and seed intake and coffee drinking, but inversely associated with fruit intake.
