(P29-038-23) Longitudinal Associations Between Physical Activity Intensities With Inflammatory Markers for Cardiovascular Disease

Objectives: Examine the long-term associations between physical activity (PA) intensities with inflammatory markers (IMs) for cardiovascular disease (C-reactive protein (CRP), fibrinogen (Fg), plasminogen activator inhibitor-1 (PAI-1), and adiponectin (AN)) in adults with and without type 1 diabetes (T1D) over three visits (~6 years of follow-up).

Methods: We conducted a longitudinal analysis of data from Coronary Artery Calcification in Type 1 Diabetes study [n=1255; T1D: n=563; non-diabetic [non-DM]: n=692]. Participants underwent physical examinations, completed the Modifiable Activity Questionnaire, and completed fasting (12h overnight fast) biochemical analyses at each visit. Mixed effect models were applied to examine the associations between the minutes spent in moderate, vigorous, or combination (moderate + vigorous) activity over the three visits. All models included a random intercept to account for patient heterogeneity and controlled for covariates listed below. Interpretation of results is defined as per ten-minute increase in intensity-type is associated with the logarithmic change in IMs.

Results: Analyses controlled for age, sex, calories, and visit (model 1) and a Mediterranean diet, systolic blood pressure, body mass index, and smoking status (model 2). In the T1D group, moderate intensity had a borderline significant association with a lower AN (-0.002 ug/mL, 95% Confidence Interval (CI) -0.004, 0.000004; p-value=0.0504). Vigorous intensity was associated with significantly higher AN (0.003 ug/mL, 95% CI: 0.0003, 0.005; p-value=0.0274). Combination intensity was associated with significantly lower CRP (-0.003 mg/dL, 95% CI: -0.005, -0.0003; p-value=0.0280) and Fg (-0.0004 mg/dL, 95% CI: -0.001, -0.00003; p-value=0.0356). For the non-DM group, moderate intensity was significantly associated with an increase in Fg (0.001 mg/dL, 95% CI: 0.0003, 0.002; p-value=0.0147) and vigorous intensity was associated with lower Fg (-0.00004; 95% CI: -0.0008, -0.0001; p-value=0.0134). PA intensities were not significantly associated with PAI-1.

Conclusions: These data suggest that PA intensities may be dissimilarly associated with IMs; however, vigorous and combination intensity are more positively associated with healthier IM profiles.
