Objectives: White blood cell (WBC) count is related to the risk of chronic diseases such as type 2 diabetes and hyperention. Therefore, the prediction of WBC count change can help manage the chronic disease risk. In the previous study, Mono-ADP ribosylhydrolase 2 (MACROD2) rs6110695 A >G polymorphism was revealed as a novel single nucleotide polymorphism (SNP) in Korean in terms of WBC count change. However, underlying mechanisms on WBC count change induced by this SNP are still vague. Thus, this study aimed to validate the clinical relevance of the rs6110695 SNP for WBC count prediction by utilizing plasma metabolites.

Methods: A total of 139 healthy Korean adults were included in the study. The rs6110695 genotype data were provided in the previous study. Plasma global metabolic profiling was performed using high-performance liquid chromatography-tandem mass spectrometry. Multiple linear regression and receiver operating characteristic (ROC) curve analyses were performed to confirm the predictive ability of WBC count prediction models using the rs6110695 SNP and/or significant metabolites.

Results: Among the 139 study participants, 129 and 10 had the rs6110695 AA and AG genotype, respectively (AA and AG genotype group). The WBC-to-apolipoprotein A-I ratio, counts of WBCs, lymphocytes, monocytes, and granulocytes, monocyte-to-lymphocyte ratio, and monocyte-to-platelet ratio were higher in the AG genotype group than in the AA genotype group. The metabolic profiling revealed that indoleacrylic acid and butyrylcarnitine were significant differential metabolites and also were significant independent predictors for the rs6110695 genotypes. Lastly, ROC curve analysis showed that the WBC count prediction model that included the rs6110695 genotype and the two major metabolites showed statistical significance.

Conclusions: This study suggests that people having the rs6110695 AG genotype with high plasma levels of indoleacrylic acid and butyrylcarnitine might have increased WBC counts. The rs6110695 genotype and levels of the two metabolites potentially provide beneficial advantages in managing the future risk of chronic diseases.
