Objectives: To compare the effects of different intermittent fasting (IF) protocols on weight loss interventions.

Methods: We systematically searched six databases, PubMed, Cochrane Library, Embase, Web of Science, CNKI, and Wanfang, up to November 2021. The included studies were double-arm or multi-arm randomized controlled trials (RCTs) on the effect of IF on weight loss, including alternate-day fasting (ADF), time-restricted fasting (TRF), intermittent calorie restricted (ICR), week-on week-off energy restriction (WOWF). Risk assessment was performed using the Cochrane Collaboration’s tool for assessing risk of bias. We systematically evaluated the intervention effects of these dietary regimens using a Bayesian network meta-analysis(NMA) of random effects. The main outcomes were changes in weight, BMI, waist circumference(WC) and fat mass, while effects were sizes of mean difference (MD). Sensitivity analyses were conducted by limiting the duration of the intervention and including more latest research.

Results: The NMA included 57 RCTs, including 3,628 participants (34.22% male) with a mean age of 43.04 ± 10.51 years. The intervention time is 2 to 48 weeks. All five interventions showed effective weight loss with mean values ranging from 2.01 to 3.26 kg. ADF, ICR, TRF, and CER showed effective interventions with MD of BMI from -0.69 kg/m2 to -1.08 kg/m2 and with MD of WC ranging from -1.41 to -2.81 cm. ADF and TRF had a significant effect on MD in fat mass which was -2.17 kg (95% CI: -3.52, -0.78) and -1.48 kg (95% CI: -2.73, -0.23). There was no significant difference in intervention effects for weight loss, BMI, WC, fat mass between different IF programs. The surface under the cumulative ranking curve (SUCRA) results of intervention ranking showed that ADF had the highest result.

Conclusions: This study is the first to compare the weight loss effects of different IF protocols by NMA. In IF, ADF is the best dietary regimen for weight loss. However, there is no significant difference compared with other programs. Our study recommended that in the clinic, it should be considered to choose a dietary regime that is more acceptable to patients.
