(P27-002-23) Probing the Cutout Point of Choline Intake To Predict Cognition Dysfunction and Alzheimer’s Diseases-Associated Dementia in a Taiwanese Case-Control Study

Objectives: Choline is required for biosynthesis of phospholipid, a neurotransmitter of acetylcholine as well as the mediator for methyl donors of folate and betaine metabolism critical to maintain neuron health and cognition function. Epidemiological study has docume Alzheimer’s diseases nted association of dietary choline deficiency with increased risk of Alzheimer’s diseases (AD), yet the optimal intake of choline to minimize AD-associated cognition impairment remains elusive. Aim of the study is to explore the adequate choline intake as to cognition function.

Methods: This study recruited 111 patients diagnosed by neurologists with the cognition function test (Alzheimer’s Disease Assessment Scale-Mini-Mental State Examination (MMSE) & Clinical Dementia Rating scale (CDRsob)) for susceptible cognition dysfunction (SCD), mild cognition impairment (MCI), and AD-associated dementia from Department of Neurosurgery at National Taiwan University Hospital. Habitual dietary choline intake was assessed by semi-quantitative food frequency questionnaire. Fasting blood was analyzed for AD pathological markers. Extracted DNA was genotyped for APOE4 allele as well as MTHFR C677T polymorphism.

Results: For MCI subjects, 73% of women and 83% of male had insufficient choline intake (< adult AI intake). After multivariable adjustment of age, gender, BMI, education, energy intake, pathological markers of blood Aβ42 and Tau and genotypes, free choline intake significantly correlated cognition function assessed by MMSE (beta: 0.93, SE:0.02, P=0.01). Only for women, cutout point of free choline intake to predict AD-associated dementia (CDR < 0.5) and cognition dysfunction (MMSE < 24) is 71.14 mg (ROC curve area: 0.70, 95%CI: 0.57-0.84, p = 0.022) and 160 mg (ROC curve area: 0.68, 95%CI: 0.52-0.85, p = 0.046).

Conclusions: Insufficient total choline intake (< women AI =390 mg/day) was associated with AD-associated dementia by CDRsob (ROC area: 0.67, 95%CI: 0.53-0.80, p = 0.0249) and cognition dysfunction by MMSE (ROC area: 0.715, p = 0.0043).
