Objectives: To determine changes in blood mineral concentrations after 8 weeks of treatment with broad-spectrum multinutrients (MN) and assess them as potential mediators of clinical treatment response.

Methods: Since the minerals Cu, Fe, Li, Mg, Mn, Se, and Zn all play significant roles in neurocognition and mood, this research explored time trends in blood mineral concentrations in participants in the Micronutrients for ADHD in Youth (MADDY) study, an 8-week double-blind RCT of MN for ADHD with emotional dysregulation in children aged 6-12. Primary outcome showed 3 times as many responders with MN (54%) as placebo (18%), rated on CGI-I at week 8. Results from 75 children (46 MN, 29 placebo) with blood minerals measured at baseline and week 8 were included in these preliminary analyses. Two-sample t-tests were used for normal data and Mann-Whitney U-test for non-parametric data to compare the mean change in mineral concentrations (week 8 – baseline) between MN and placebo groups. If significantly different between the 2 groups, change in blood mineral levels were evaluated as potential mediators by including an interaction term with treatment group in a logistic regression model with outcome of CGI-I treatment response.

Results: Blood concentrations of 3 minerals (Li, Se, and Zn) increased significantly in response to MN compared to placebo after the 8-week RCT (i.e. Li [z=5.750, p< 0.001], Se [t(73)=2.32, p=0.02], Zn [t(73)=2.05, p=0.04]); however, there was no evidence the changes mediated clinical treatment response. Changes in Cu, Fe, Mn, Mg were comparable between groups.

Conclusions: Blood concentrations of Li, Se, and Zn increased with 8 weeks of MN compared to placebo; however, these changes did not predict who responded to the treatment. Further formal mediator analyses will be conducted modelling the full intention-to-treat data. Li, Zn, and Se should be considered in future hypotheses to identify biological mechanisms of action for a multinutrient supplement for ADHD and irritability.
