Objectives: To evaluate the association between low-carbohydrate dietary patterns and cognitive function among older adults in China.

Methods: Dietary intake was assessed via a validated food frequency questionnaire (FFQ) in 2014, and low-carbohydrate diet score was calculated. Cognitive function was assessed by Mini-Mental State Examination (MMSE) administrated between 2012-2016. Multivariable linear regression models were developed to examine the association of dietary patterns with cognitive function, adjusted for socio-demographics, major lifestyle factors, chronic diseases, and total energy.

Results: Individuals with higher low-carbohydrate diet score, which indicated high protein and fat intake but low carbohydrate intake, had worse MMSE scores (adjusted mean difference for the highest compared to the lowest quartiles: -0.87; 95% confidence interval (CI): -1.1, -0.63; P-trend < 0.001).

Conclusions: An inverse association between low-carbohydrate diet score and cognitive performance was observed, suggesting that higher dietary carbohydrate intake may play a protective role against cognitive decline in older adults.
